COMPREHENSIVE EVALUATION – 1.5 HOURS

A complete evaluation includes sleep education, a review of your history and creating an individualized plan for your child and family.
Once your evaluation appointment is set up, I will send you a client information and developmental history form via fax or email. This form, although lengthy, allows me to get a 24-hour snapsho
t of your child's current situation. This information is vital to our creating an initial plan that is tailored to your child's needs.
I ask that you return the completed history to me (via fax or email) 3 days prior to your evaluation so that I may read it before the evaluation. The initial evaluation is approximately 1.5 hours long with (ideally) both parents over the phone or in my office in Annapolis, MD. I will review your completed history form prior to our evaluation. The evaluation consists of sleep education; gathering further information regarding your history, and the creation of an initial plan to improve your child's sleep habits.

EIGHT (8) COACHING PHONE FOLLOW UPS (approximately 3 hours of one-on-one phone contact with The Sleep Lady)

These 8 phone follow-ups may be added to the 45 minute evaluation or the 1.5 hour evaluation. I call you to review your sleep log, provide additional support to you, answer your questions and discuss possible changes to the initial plan. Usually these follows up are completed within 2 to 3 weeks of starting our sleep plan. Additional phone follow ups can be purchased if needed.
I ask that parents pay for the complete package at the initial evaluation time. I accept Master Card and Visa. If you wish to pay by check, please mail me a check so that it arrives by the evaluation appointment (my address is available on the contact page of my website). I will not deposit your check until after the evaluation has been completed.
Upon completion of our work, I will mail you a detailed receipt containing all of the information you will need for possible insurance reimbursement. Insurance reimbursement, in most cases, is limited to the office visit portion of the services provided. Most insurance plans do not cover phone consultation services. If you are interested in submitting this to your insurance company, call your insurance company and ask about your outpatient, out-of-network mental health benefits. You may also want to ask how to file a claim for reimbursement directly to you.
