INDIVIDUAL CONSULTATIONS

1. COMPREHENSIVE SLEEP SOLUTION PACKAGE WITH THE SLEEP LADY [$750]
A complete evaluation which includes sleep education, a review of your history and creating an individualized plan for your child and family with 8 follow up phone calls as outlined below.

Once your evaluation appointment is set up, I will send you a client information and developmental history form via fax or email. This form, although lengthy, allows me to get a 24 hour snapshot of your child's current situation. This information is vital to our creating an initial plan that is tailored to your child's needs.

I ask that you return the completed history to me (via fax or email) 3 days prior to your evaluation so that I may read it before the evaluation. The initial evaluation is approximately 1.5 hours long with (ideally) both parents in my office in Annapolis, MD or by phone. I will review your completed history form prior to our evaluation. The evaluation consists of sleep education; gathering further information regarding your history, and the creation of an initial plan to improve your child's sleep habits.

This comprehensive package includes 8 phone follow-ups where I call you to review your sleep log, provide additional support to you, answer your questions and discuss possible changes to the initial plan. Usually these follows up are completed within 2 to 3 weeks of starting our sleep plan.

I ask that parents pay for the complete package at the initial evaluation time. I accept Master Card and Visa. If you wish to pay by check, please mail me a check so that it arrives by the evaluation appointment (my address is available on the contact page of my website). I will not deposit your check until after the evaluation has been completed.

2. ONE TIME 45 MINUTE CONSULTATION WITH THE SLEEP LADY FOR FAMILIES WHO HAVE READ "GOOD NIGHT SLEEP TIGHT" [$225]
This option is for parents who have read my book "Good Night Sleep Tight", have started the Sleep Lady Shuffle and wish to set up a 45 minute consultation to ask specific questions on the recommendations outlined in the book. I ask that you read the first two chapters and the age appropriate chapter to your child prior to our consultation. The fee for this consultation is $225 and includes my reviewing your history prior to our consultation. I will ask that you fill out this history form as outlined above. I will not be providing any sleep education during this time but will answer your specific questions and make recommendations on tailoring the Sleep Lady Shuffle to your child and family.

If you wish to set up either an evaluation with the phone follow ups or a 45 minute consultation, please call my office at 410-974-1600 and leave a message with your name with day and evening phone numbers my assistant, Tammy, can call you at.

Upon completion of our work, I will mail you a detailed receipt containing all of the information you will need for possible insurance reimbursement. Insurance reimbursement, in most cases, is limited to the office visit portion of the services provided. Most insurance plans do not cover phone consultation services. If you are interested in submitting this to your insurance company, call your insurance company and ask what your outpatient, out of network mental health benefits. You may also want to ask how to file a claim for reimbursement to you.

Answers to other frequently asked questions:

-Most of the children I work with are newborns to 5 years old. No, it's never too late!

-I address naps as well as going to sleep at night and staying asleep.

-Yes, I have worked with many multiples and often speak at their meetings.

-The phone follow ups are not required; however, most parents wish to have them in order to help them be more consistent and to have a supportive, experienced ear to brainstorm with.

-I have worked with many out-of-state families strictly over the phone.

-I can work with you over the phone if you are outside of the United States.

-The "Getting Your Newborn Off to A Good Sleep Start" phone class is offered 4 times a year. This class covers newborns to age 5 months and is set up to help you prevent sleep problems. See details about the next class.
